DERMATOLOGY CONSULTATIONS

Background and design: The aim of our study was to evaluate the reasons of dermatology consultations in inpatient departments.
MATERIAL-METHODS: A total of 269 patients consulted over a period of six months from the different clinics were included in the analysis.
RESULTS: Consultations were requested mostly from internal medicine (64), emergency (34), pediatrics (23), the most common dermatological diseases diagnosed by consultants were infections, eczema, urticaria, drug reactions and ulcerations. Consultations requested from emergency services constituted 13% of total consultations and only one third of them were true dermatological emergencies. Local therapy was prescribed in 81.3% and systemic therapy in 43.1% of the patients. 10.4 % did not receive specific therapy.
CONCLUSIONS: Nondermatologists are enable to diagnose even common dermatoses like acne and psoriasis which cause an increase in the number of consultations and affect the quality of dermatology consultations. Diagnosis and management of common skin diseases, skin manifestations of systemic diseases and true emergencies should be stressed during the training and teaching of nondermatologists.

DERMATOLOJİ KONSÜLTASYONLARI

AMAÇ: Bu çalışma yatan hastalarda dermatoloji konsültasyonlarının sebeplerini araştırmak amacıyla yapıldı.
GEREÇ-YÖNTEM: Altı aylık sürede farklı kliniklerden konsülte edilen toplam 269 hasta değerlendirmeye alındı.
BULGULAR: Konsültasyonlar en çok iç hastalıkları (64), acil servis (34), çocuk hastalıkları (23) servislerinden istenilmiş olup konsültanlar tarafından en sık tanı konulan hastalıklar enfeksiyonlar, ekzema, ürtiker, ilaç erüpsiyonları ve ülserasyonlardı. Acil servisten istenilen konsültasyonlar tüm konsültasyonların %13’nü oluşturuyordu ve bunlardan sadece üçte biri gerçek dermatolojik acillerdi. Lokal tedaviler %81.3 ve sistemik tedaviler olguların %43.1’inde önerildi. Olguların %10.4’ünde spesifik bir tedavi önerilmedi.
SONUÇ: Dermatologlar dışındaki hekimler akne ve psoriasis gibi sık görülen hastalıkların bile tanısını koyamamaktadır. Bu da konsültasyon sayısının artmasına ve dermatoloji konsültasyonlarının kalitesinin azalmasına yol açmaktadır. Dermatologlar dışındaki hekimlerin eğitim ve öğretimlerinde sık görülen deri hastalıklarının, sistemik hastalıkların deri belirtilerinin ve gerçek acillerin tanı ve tedavisinin vurgulanması gerekmektedir.
