Narrowband UVB therapy in the treatment of lichen planus

Background and design: Recently, UV-B lamps with many immunologic effects and a peak emission around 311 nm have been used successfully for the treatment of many dermatologic diseases, known to be treated with PUVA. Narrowband UV-B (NB-UVB) radiation causes less erythema and carcinogenicity with lower cumulative doses than PUVA, while the treatment response remains high. Lichen planus (LP) is a cell-mediated immune response of unknown origin. Materials and Methods: We present our results of NB-UVB therapy given to ten LP patients. The sessions were administered 3-4 times weekly with an average cumulative dose of 17.7±1.6J/cm2. Results: Five patients responded completely, four were partially responsive at the end of 30th session. Three of the partially responsive cases responded completely at the 51th, 31th and 36th sessions, respectively.
Conclusion: Clinical improvement observed in our study, imply that NB-UVB therapy is an inevitable treatment alternative for resistant cases of LP.

Liken planusta dar bant UVB tedavisi: Ön çalışma

Dar bant UVB tedavisi (311nm), immunolojik etkileri nedeniyle son zamanlarda PUVA ile tedavi edildiği bilinen birçok dermatozda uygulanmaya başlanmıştır. Dar bant UVB ışını (DB-UVB), PUVA'ya kıyasla daha düşük kümülatif dozlarda, dolayısıyle daha az karsinojenite ve eritem riski ile aynı etkiyi gösterebilmektedir. Liken planus (LP), hücresel immün yanıtta bozukluğun söz konusu olduğu nedeni bilinmeyen bir dermatozdur. Bu makalede, DB-UVB tedavisi verdiğimiz on LP hastasında elde ettiğimiz sonuçları sunuyoruz. Tedavi, haftada 3-4 seans ortalama kümülatif doz 17.7±1.6J/cm2 olacak şekilde uygulandı. Otuzuncu seans sonunda, beş hastada tam, dört hastada ise kısmi yanıt elde edildi. Kısmi yanıt alınan dört hastanın üçünde sırasıyla 51., 31. ve 36. seanslarda tam yanıt alındı. Çalışmamızda elde ettiğimiz klinik yanıtlar, DB-UVB'nin, özellikle dirençli LP hastalarında, etkili ve gelecek vaat eden bir alternatif olabileceğini düşündürmektedir.
