Pemfigus is an autoimmune bullous skin disease, characterized by intraepidermal blisters. It is a severe and potentially life-threatening chronic disease with blisters and erosions on the mucosae and the skin. Treatment options do not differ for two most common types of pemphigus, pemphigus vulgaris and pemphigus foliaceus, except that the latter is usually less resistant to treatment and corticosteroids can often be started at lower doses. Systemic corticosteroids are still the most widely used drugs in the treatment of pemphigus and continue to be the mainstay of therapy for this disease. Adjuvant drugs are commonly used in combination with the aims of increasing efficacy and of having a steroid-sparing action, thereby allowing reduced corticosteroid side-effects. Mortality and complete remission rates have improved since the introduction of adjuvant drugs to pemphigus. Adjuvant drugs include immunoadsorbtion, corticosteroid pulse therapy, intravenous immunoglobulin (IVIG), immunosuppressive agents such as azathioprine, cyclophosphamide, mycophenolate mofetil and and anti-CD20 monoclonal antibody (rituximab). The lack of consensus in the published literature about the treatment of this disorder is responsible for different treatment strategies. Treatments need to be chosen after careful consideration of the potential benefits and side effects according to the patients’ medical condition. Pemfigus intraepidermal büllerle karakterize otoimmün büllöz bir hastalıktır. Deri ve mukozaları tutan, hayatı tehdit edebilecek kadar ağır seyredebilen bir hastalıktır. Pemfigusun en sık görülen 2 tipi olan pemfigus vulgaris ve pemfigus foliaceus için tedavi seçenekleri aynıdır fakat ikinci hastalık daha hafif olduğu için daha düşük kortikosteroid dozları ile tedaviye başlanabilir. Sistemik kortikosteroidler en sık kullanılan ajanlardır ve tedavinin temelini oluştururlar. Kortikosteroidlere eklenen adjuvan ajanlar etkinliği artırmanın yanında kortikosteroid kümülatif dozunu buna bağlı olarak da kortikosteroid yan etkilerini azaltma amacı güderler. Pemfigusda adjuvan ajanların kullanılmaya başlanılması ile mortalite oranlarında azalma tespit edilmiştir. Başlıca adjuvan tedaviler intravenöz immünoglobülin (İVİG), immünoadsorbsiyon, kortikosteroid puls tedavisi ve azatiyoprin, siklofosfamid, mikofenolat mofetil ve anti-CD20 monoklonal antikor olan rituksimab gibi immünosüpresif ajanlardır. Pemfigusda en ideal tedavinin hangi tedavi olduğu konusunda hala bir konsensus sağlanamamıştır. Çünkü az sayıda prospektif randomize kontrollü çalışma mevcuttur ve bunların da tedavi ve iyileşme kriterleri ve sonuçları birbirlerinden farklılıklar göstermektedir. Tedavi hastanın durumuna göre kar-zarar oranları dikkate alınarak planlanmalıdır. Bu derlemede pemfigusun klasik tedavileri ve yeni tedavi yaklaşımları tartışılmıştır.
